iPhones only allow notifications from Home Screen apps. In Safari tap Share (the square with the arrow) → Add to Home Screen → Add. Then close Safari and open Push Test from your Home Screen.
After tapping 2, lock your phone. A notification should arrive within about 15 seconds.
